Physical Exam Form 

NYSED requires a physical exam for new entrants and students in Grades Pre-K or K, 1, 3, 5, 7, 9 & 11; annually for interscholastic sports; and working papers as needed; or as required by the Committee on Special Education (CSE) or Committee on Pre-School Special education (CPSE). 

• A new form signed by the medical provider in reference to Allergy, Asthma, Diabetes, or Seizure Action Plans and the Parental Consent Form are required at the beginning of every school year. 

• Medications must be brought to the school health office by a Parent/ Guardian ONLY!  

***PLEASE DO NOT SEND MEDICATION IN YOUR CHILD’S BOOKBAG FOR THE SAFFETY OF ALL STUDENTS! *** 

• Prescription medication must come from the pharmacy in its original unopened, labeled container. 

Medication Drop-Off  

The Health Office will be open beginning August 20th, 2025, from 8:30 a.m. to 3 p.m. 

Children attending day care and pre-K through 12th grade in New York State must receive all required doses of vaccines on the recommended schedule in order to attend or remain in school.   

Important School Immunization Information  

Within 14 days of the first day of school or day care, parents must: 

• Show proof of their child's up-to-date vaccinations, OR 

• Provide proof of a scheduled appointment via an appointment card  

In order to attend or remain in school or day care, children who are unvaccinated or overdue must receive at least the first dose of all required vaccines within the first 14 days. They also must receive subsequent vaccines in the series within a 14-day period of when they are due to complete the immunization series.
